I think the judge is correct. When there is an order for child maintenance pre-dating 2003 the CSA has no jurisdiction, the court order remains in effect until a variation application is made and the court dismisses the order or it is discharged.

Not going through the CSA.

I now understand I have to submit a FORM A, and also confirm that an FM1 is not goingto work as my ex refuses to go to mediation.

Is there anything else I need to file?

No, you just need to include the court fee.
