can someone advise what form(s) I need for the above please?

My Directions Hearing yesterday in relation to the FH that took place in June went as well as I expected once it was clear that the same judge who screwed up at the FH would be the fella we would see again at the DH: the only good thing was that I self repped and therefore didn''t pay another barrister stupid money to get the same negative result.
