Hi I got my notice from the court last month, first hearing in August and they’re not including the forms now. They tell you which ones you need and you have to download them. I’ve had the same issue as the above poster, having to look for the right forms myself. You also have to file them and all of your evidence electronically via email. 3 hours of scanning bank statements to do this week. May be specific to my court which is Nottingham.

There aren't forms for the statement of issues and chronology.

You need a header - court, case number and names of applicant and respondent.

Then a title eg. Applicant's Chronology

The chronology is sn ordered list of all key dates relating to the marriage and finances. So date and event.
List things like dates of birth of the parties and any children, start of cohabitation, marriage, separation, petition etc. Include dates of putchase of property, receipt of inheritance etc.

The Statement of issues is a simple numbered list of the things you need the court to help sort out. This csn be everything from the value of the FMH if you disagree, through to how housing needs should be met and pensions treated.
