My girlfriend has now received her ex's Statement of Issues and his questionnaire. They are asking for all kinds of proof/documentation. My question is, given that the first appointment is a phone conference, do we collect all this together and post it off again to them and the court along with the form H?

You don't need to answer the questions until after the hearing. The judge will set a timetable for responses and perhaps also supplementary questions.

If there are missing documents that can easily be provided there is no harm in sending those ahead of the hearing.

If there are questions that are inappropriate or irrelevant you can ask the judge to strike them out.
