What exactly does the order from the court say??

It is usual for all signed court documents to be submitted as PDFs. As you have pointed out, the online form E is a PDF. If you handwrote and scanned it in it would not be a word doc.

The court may request a draft order in word format which allows it to be altered by the judge as needed.

It may be polite to provide a word as well as pdf version of your questionnaire to the other side to make it easier for them to write their responses into the document next to the questions.
