Any advice on what is appropriate here? I don't want to give out unneccessary details on the form.
My daughter has a CAMHS referral for self-harm but doesn't want her father to know about this, and my son is waiting for treatment for anxiety and I would like to pay for some private therapy for him. Father has agreed to pay for half, but like many things he has said he would pay for, I doubt the money will ever be paid out.

My youngest had a medical condition which I didn't want ex to know about so I wrote 'currently being monitored under hospital specialist care'

Courts and judges don't even ask or question it.

And the questionnaire only includes questions regarding financial disclosures.
