Confused over the D80A form

Hi, I was wondering if anyone could help me?:S

I want to apply for a Decree Nisi. I have received an ''acknowledgement of service'' form signed by my wife from the court, along with a D84 and a D80A form.

I have no problem completing the D84 form but I am confused about the D80A form. The version I have been sent by the court is version ''04.12'' dated 2012, whereas the D80A form available for download from this site is version ''04.11'', dated 2011. The form''s title has changed from ''Affidavit in support of divorce...'' to ''statement in support of divorce...''. The page for the Affidavit no longer exists in the new version, it simply has a ''statement of truth'' page.

I will use the D80A form sent by the court but do I still need to get the D80A form witnessed and sworn by a court / solicitor? Or do I have to supply a different form along with the D84 and D80A forms that contains the sworn Affidavit?

They tinkered with the forms in the hope of confusing everyone for no apparent reason, they changed titles not that anyone has adopted them, not even judges, this one is particularity good its not an affidavit in support anymore its a statement of truth, even though it is an affidavit :angry:

Anyway to answer your question yes it must be sworn, words changed but the process remains the same.

where is the sworn recorded as there is no section for it on the new form, only a section for me to sign?

You sign it then take it to court, the swearing part is done before an officer of court, you just read from a card once done they stamp the forms so the judge knows they are sworn.

can I do this in front of a solicitor and then simply send the paperwork to the court?

Hi,
I had these forms two weeks ago,but didnt see anywhere that said i had to take them to the court to be signed before a court official.
So i just signed them and sent them back!:(
Not had them sent back yet,so will just keep my fingers crossed!!

This is an affidavit it mist be sworn if not it will be returned and it will cause a delay.
