Completing a D80B

Hopefully someone can help me! I am currently completing a D80B. I only received the D10 from the Respondant but should I have receieved anything else?
I filed for divorce in March 2013.
The information in the Petition is based on October 2012, however I also refer to information brought to my attention in January 2013. So what date do I specify in Q5? He left the family home immediately so no behaviour is continuing.

Lastly, I asked for him to pay the court costs to which he says he can''t should I just accept it and leave it at that or tick the box at the end of the D80B?

When I file this to the court is it 3 copies again and do I just have to attach the D10 (acknowledgement of service)or should there be other docs too?
