I served the forms on my ex 5 weeks ago, application for contact order, Amongst the bundle from the court to me was a form of a couple of pages amongst others which I served her, (through sol) the respondent is supposed to fill this form in and return a copy to me and the court, its asks basic identity stuff but also about if she intends to defend the case etc, I have had no form back to me, she is supposed to give me a copy and the court, my case is heard tomorrow but i cant remember the form number to ask for on the day, can anyone help?

It is her acknowledgment. They are rarely completed, wouldn't let it concern you.

strange you should say that, why include it? Surely if court etiquette is generally adhered to, then why not treat all forms the same? As much as I have to serve her and I strived to get a witness statement in a few days ago to avoid them adjourning the hearing. thanks for your reply anyhow amanda
