I am filling in the form to go to court to apply for Decree Nisi, and have realised my married name has always been spelt wrong on my marriage certificate! Could this cause a problem?

It can be yes so add a note to explain the spelling mistake when you send in the D84, its best to staple the note to the form so it does not get lost.
