Form D180 - need help with some questions

I am preparing form D180 which I need as the marriage was taken in another EU country (which I am national of), but divorce took place in UK.

What do I put in the box on top after the:
" In the .... "?

Point 1 - Memebr state of origin - is it where divorce took place?

Point 2 and Point 4 - is this the same Court information? So is the Court issuing the certificate the same as Court which delievered the judgment?

Point 6 -legal aid - if all was paid for by us, is it: N/A ?
