what court form do I use ?

I went to court last May. I was a self repper. My Ex had a barrister. The Judge decided that my Ex should be given more time to sort out the finances. A court order was written and we both had to agree it under oath in court.

Whilst we were in court I asked for the subject of access to my property (jointly owned but my Ex lives in) to sort out my possessions and personal stuff (he still has my birth certificate etc). It was agreed in court that he would allow me access to the house, he would leave the property and turn off all surveillance equipment . He was told he could arrange for a solicitor to be in the house if he wanted.

A year later despite me asking many times I have not been let in. He solicitor will not even acknowledge my emails of request to go back into the house. The Judge even provided us with a transcript of the Court hearing in case what was agreed got forgotten.

The Judge did grant our Decree Absolute on the day last year. Which as we have lots of joint accounts in debts in causing lots of issues but the barrister said he thought it was for the best.

I am now wanting to request extra directions or take my ex back to court to get entry to the house. I also believe that he has broken the court order but my mental health is at an all time low which he knows - and he tells me if I fight it in court I will lose like lastime. Which court form do I use ?
