D11 - Deemed service

Hi,

I''ve been informed by the court that I need to fill in a D11 asking for deemed service but would like some clarification as to what information I need to include.

Although I have no idea where my husband is, email communication from him says that he has seen and signed the documents, but hasn''t bothered to send the Acknowledgement of Service back to the court, hence the advice to send a D11.

I have applied for deemed service, given a written explanation in the appropriate box and printed out the emails as evidence to attach, but would like to know who should be served with the application, please. As previously mentioned, I have no idea where my husband is, although I could give his grandmother''s address, and don''t know whether or not I should include my own details in this box.

Any help would be gratefully received!

yes it's required. Give the respondents full name and address including post code and telephone number if you have one.
