Hi
I need to fill the form. I got stuck on the 3rd page at the top.
There are 2 questions.

1) [ ] I identify the signature appearing in paragraph 10(a) of the Acknowledgement of Service, a copy of which I have attached to this statement and marked ‘A’, as the signature of my [ ] spouse [ ] civil partner, who is the Respondent in these proceedings.

2) [ ] I identify the signature appearing at the foot of the document which I have attached to this statement and marked ‘C’ as the signature of my [ ] spouse [ ] civil partner, who is the Respondent in these proceedings.

The first one understand. I will put the mark C next to my wife signature but 2nd has confused me. Which document do I need to attach? where do I need to put the mark C.

Please I need a help.
