D80B help!

Hi everyone,
I need a little help filling in the D80B form.....
Im on page 3, where it says \"I [my name] of [my address] make oath and say as follows...\"

And number 3 says \"I identify the signature........appearing on the copy acknowledgement of service now produced to me and marked A as the signature of my husband/wife, the respondant in this cause.\"

But he wouldn't sign the divorce papers!
I had the bailifs go round to serve the papers, and they then sent me a form (it says D90 on the corner, but I cant find that online).
It says that a copy of the divorce petition was served on whatever date, and says ID of the respondent was established by receipt of the signature of the respondent as follows...
Then there is a scribble, which I do not recognise!

The bailiffs wrote something at the top of the form which I can't read properly, but looks like \"Left He....[?]. asked next door not in poss empty\"

So, my question is, do I just leave that line empty on the D80B form?
