Help with form D11

The court bailiff has been sent to serve the D10 papers and I know 100% that my husband wont sign - even though we have lived apart for 7 years and there is nothing he can do about it. Child access order is in place. I am told the next process is to fill in form D11.
Question 3 on Form D11 asks: What order are you asking for and why..........I know this sounds stupid but has anyone any idea what to put here? What I want is the divorce to go ahead without him signing the D10 form as he wont sign, is that what I put, any ideas?
Also I don''t understand question 10: What information are you relying on.

Sorry to be so dim but I''m getting bogged down with all the different forms, and just when I think I have cracked it, up pops another form!

The order you are asking for is Decree Nisi.

You are relying on proof of service from the bailiff which you attach to D11

Yes if they catch him in they give you a form saying where and when he was served, staple it to the back of D11.

Ps process servers tend to be much more successful that court bailiffs.

I am also at the D11 stage!HELP!!

court bailiff tried to serve and his flat mate informed him that he does not reside at the property, but he collects his mail regularly! Proof? Nevertheless, my papers have not been served by the bailiff!
I have proof that he has received the original papers that were sent from the court. Conversations through text messages, blatantly stating that he is not signing my F***ing paper’s, photo’s as he stupidly had them displaced in his car that I accidently took photos off J when I dropped of my children for contact with him.
I also do not qualify for legal aid and I also cannot afford a solicitor.

I have been advised to complete D11 for “ Deemed service”
D11 Q3 I have been advised to write "Deemed service of the divorce petition"
Attaching the evidence that I have to prove he has received such as photos, text transcript and court bailiff response to state that he regularly collects his mail.

Is this correct? As I am unsure now from reading different posts.

I have been advised that at question 3 that I put ''A decree Nisi.'' Perhaps we should put both - Decree Nisi and Deemed Service?? Help!

I can''t thank you enough for taking the time to reply, I couldn''t do all this on my own. To late to have the process servers as the request for court bailiff has already gone in. I''m just trying to get one step ahead by sorting out the D11 form ready to send in. This form has given me the most trouble - thanks again Vickyd
