Top right hand box is NOT to completed by you.

Once completed, we had to submit the D180 form to the same county court where my now-husband had had his divorce papers issued.
We spoke to the court reception by phone beforehand to confirm the procedure and we had to pay 60 pounds. (They took payment over the phone from my credit card and sent me the receipt along with the signed documents).
Just to be on the safe side we also attached photocopies of the decree nisi and decree.
From submitting the form (we did so by registered post) to receiving the signed copy back took 4 weeks as they told us the judge was always very busy!!

I would suggest to play it safe and definitely contact the county court prior to sending any documentation.  The information I’ve shared on the process we had to follow is back from 2016, so possibly things may have changed.

Prior to this, I had actually emailed the court directly because I wanted in writing, exactly what documentation needed to be submitted.  I simply Googled the county court to find their phone number & email address.

This was their reply:

The court would require a completed D180 (Article 39) and fee of £60 in order to process and certify the Decree Absolute as requested. The D180 form can be downloaded from website www.justice.gov.uk under forms and guidance. The court accepts payment of the fee by cash, postal orders and cheques payable to HMCTS.

Good luck!
