My ex to be wife doesent want MIAM whatsoever. How I can complete form A please? Do I apply with exception section 3d ``the prospective applicant does not have sufficient contact details....for the purpose of scheduling the MIAM`` ?

Refusal to attend MIAM would mean you attend alone to get the sign off..

Is she in the UK? Is she still running the business?? What address did she use for the divorce?? You will need to give an address for the Notice of First Appointment to be sent to. Otherwise you may have to serve it, using a process server if necessary, and provide proof that she has it.
