Hello, like so many others in this forum I am needing to complete the D180 Form and like everyone else I am confused by the lack of instruction / clarity on the Aswers required.

Firstly Marriage, I presume this to be what is on the original marriage certificate but no clues given, so
3.1 Wife
3.1.1 Full Name : Is this Maiden Name as per Wedding Certificate.

3.2 Husband
3.1.2 Address : Again is this the address he had at time of marriage as stated on our Wedding Cert.

Thank you
