PENSION SHARING ANNEX - CAN SOMEONE HELP PLEASE?

I am representing myself through my divorce. I have already received, completed and subsequently returned the pension sharing Order and Annex to my stbx wifes solicitor. Due to the fact that I wanted us to share the cost of the set up fee of the pension sharing, she is not. hence the fact i now have another pension sharing annex sitting on the mantlepiece. can someone help me fill the blank form in (the previous one was filled in by her solicitor) and do I simply return it to her solicitor or to my pension administators (who are not very helpful on the phone at all). any advice would be most appreciated

Top left rule out "paragraph 15 of the schedule bla bla"

You are the transferor

Your ex is transferee

Under take notice rule out

"Varied discharged an order bla bla"

Put N/A in the boxes that don`t apply

Section D

Rule out *The pension sharing charges bla bla"

Leave the charges to be paid in full by you bit underneath.

Section E

If you have completed form D81/M1 then yes (tick box)

Section F-G-H-I-J N/A as your pension is uniformed service, but check with the provider

Last page rule out the civil partnership bit leaving the first two lines.

This is an order for your pension provider so send it to them, as you are doing this alone without a lawyer photocopy it and send a copy to the other sides solicitor by recorded delivery.

And well done you completed the form rather quickly ever thought of a career in family law B)
