Child Contact and Residency
Ask for permission to start proceedings, or to request an order or directions in existing proceedings, or to ask to join or leave proceedings.
Apply for any order under the Act except care and supervision orders, Section 8 orders and enforcement of a child arrangements order.
This form is to be used by any party in Family Proceedings who does not wish to reveal their contact details (private
address, telephone number, email etc.) or the contact details of any child. These details will not be revealed to any person
except by order of the Court
Application for attachment of a warning notice to a child arrangements order
Record an agreement between a child’s parents and a step-parent (who is the spouse or civil partner of a parent) that the step-parent should have parental responsibility for the child.
Parents can use this form to record an agreement that the biological father should be granted parental responsibility for the child.
