Thanks to the information posted here I think I have enough to be able to provide the court with the correct details - but, as previously said, none of the wiki-library links on the site seem to work.

We are aware that there are library links that need updating and our volunteer team are working on this.

Form G is only available from the court, and is included in the papers sent from the court notifying a person of the first AR hearing. The form is not available for general download.

It's a very simple form which you can knock up yourself (if you know what it looks like). Thankfully some helpful barrister's chambers prepared their own copy: www.kentbarristersdirect.co.uk/doc/Form_G.doc

This would be really helpful, to have an actual Form G as an exemplar, but the one listed here took me to a copy of a Notice of Response To First Appointment for FDR.
