How to complete a D11 form

Hi everyone,
I'm in need of support to help me complete a D11 form. I started the application earlier this year and my wife has confirmed she has received the application. However, as things are not amicable between us, she messaged me stating that she could not respond by the set date (15th Feb) as she was wanting to speak to her solicitor and this would not be until early March. I have now started to fill in the D11 form as I have evidence (text message) from her stating she has received the application. My problem is what to put into each of the sections in the form.

Can anyone provide me with instructions for me to follow please.
